Noticeboard

Following the Welsh Government announcement about a lock-down on Friday, please be advised that our flu clinic at Bridges Community Centre on Saturday, 24th October will go ahead.  Those patients who are booked into the flu clinic may travel to Bridges as it is deemed an essential journey.

This year demand for the flu vaccination has been particularly high; if you didn't manage to book a Flu Clinic appointment, please now contact us to add your name to our waiting list.  We will contact you as soon as we have news about additional stock.

Many thanks for your patience

New Patient Registration

To register at Dixton Surgery you have to complete some registration forms, a new patient health sheet and have a New Patient Health Check with a nurse.

Entitlement to NHS Services may be questioned in some circumstances.

Practice Area

Our practice area is clearly defined on a map at the surgery. Roughly, we reach as far as Goodrich to the north of Monmouth, Staunton to the east, Raglan to the south and Cross Ash to the west. If you are in any doubt please ask to be shown the map.

Change of Address

We ask that you let us know of any change of address and for other members of your family unit. If you fail to do so and, for example, a referral is made for you to see a consultant, the hospital’s invitation will most certainly go to your previous address and you will miss your appointment.  You might also miss routine invitations for Screening (Breast, Bowel etc) and so it is very important that we have your correct address.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
