PLEASE GIVE US AN UP TO DATE MOBILE PHONE NUMBER - we are starting to use text messaging to remind patients to book health/medication reviews. If you DO NOT wish to receive texts, please let us know and we will be happy to remove you from this service.
We hope our website makes it easy for you to gain access to the information you need. As well as specific practice details such as opening hours and how to register, you’ll find pages covering a wide range of health issues along with links to other relevant medical organisations.
We’re not just here for when you are unwell. Our team of healthcare professionals and community staff offer a number of services to promote good health and wellbeing, whatever your medical condition.
We are a training and a teaching practice, and as such welcome GP Registrars and Medical Students to our practice team.
We aim to provide the best possible care that we can. Your helpful comments and suggestions are a good way to help us to monitor and improve our services and so if you have a helpful and constructive comment, please let us know via our feedback function.
PLEASE NOTE THAT HEALTH RELATED QUESTIONNAIRES ON THIS WEBSITE HAVE A PRINT ICON AT THE TOP RIGHT HAND CORNER. IF YOU CHOOSE TO COMPLETE A HEALTH QUESTIONNAIRE, PLEASE FEEL FREE TO PRINT YOUR COMPLETED QUESTIONNAIRE AND SEND IT TO US BY POST IF YOU PREFER. YOU DO NOT HAVE TO SUBMIT YOUR RESPONSE ON-LINE.
Access to the surgery and waiting area is via a side door of the building. For patients with limited mobility or who require wheelchair access, please use the disabled access door further along the path at the side of the building. Please ring the door bell and a member of staff will show you to the waiting area, consulting and treatment rooms and disabled toilet facilities. When making an appointment or contacting the surgery we would be grateful if you could remind the staff that you would like to be let in through the disabled access door.
Change of Address
We ask that you let us know of any change of address for you and for other members of your family unit. If you fail to do so and, for example, a referral is made for you to see a consultant, the hospital’s invitation will almost certainly go to your previous address and you will miss your appointment. You might also miss routine invitations for Screening (Breast, Bowel etc) and so it is very important that we have your correct address. Please let us know when you attend the surgery, or alternatively complete the on-line form by using the link below.
